The clitoris can be made
sensitive thanks to the arrangement of the blood and nerve supply to the head
of the penis. This is separate
from that to the skin, or the two cylinders of erectile tissue (the corpora)
and a part of the sensitive tip of the penis can therefore be dissected out as
an island flap, and used for the clitoris.
This is shown in the diagram below, which shows the construction of a
neoclitoris in a patient using the penoscrotal flap technique.
Like all flaps, the blood
supply may fail, but a sensitive clitoris is produced in over 90% of cases.
