A document like this needs to be witnessed
preferably by a commissioner for oaths at your local solicitors. It can cost anything between
£5 and £20 for this service, or you can visit your county court witnessing
service.
If you want copies made to send to various official bodies like the passport
service and so on, then it is wise to print a few of these out, ensure each one
is signed in front of your witness (not copied).
This below is for guidance only it is always
better to have this on the solicitors headed paper - as it looks more
official to lay people. It may also be expected by your gender specialist -
home made 'StatDecs' can look suspicious even if they are legal.
